Step-by-step guidance for completing a shift handover form so keys, access, and outstanding tasks transfer with clear acknowledgment.
A shift handover form is for accountable transfers — keys, access, an incident log, or a role — between two named people. Here is how to fill one so it holds up later if someone asks who had what.
1. Name both parties and the reason
Write who is handing over to whom, with roles, and why (end of shift, change of role, leave). Vague "usual handoff" lines are useless in a dispute.
2. List items and access individually
Do not write "usual equipment." List keys, radios, fobs, and credentials one by one. If it can be contested later, it belongs on its own line.
3. Capture outstanding tasks
Anything the incoming person is now responsible for — with a deadline if you have one. This is where forms beat chat: the task travels with the acknowledgment.
4. Get both acknowledgments
A verbal "got it" is not a form. Use signatures, initials with dates, or an in-app acknowledgment tied to a named account before the outgoing person leaves.
