Shift Log Template

Use this template to log events as they happen during a shift — the raw record a handover or report gets summarized from.

Log events as they happen, not from memory at the end of shift

One row per event — don't batch multiple events into one line

Flag follow-up items so they're easy to find later

What to include

  • A timestamp for every entry, not just a shift-level summary
  • Who logged the event, for accountability
  • What action was taken at the time, not just what happened
  • A flag for whether the entry needs follow-up, so it's scannable at a glance

The template

Logged byR. Patel
Notes / action takenSigned for delivery, discrepancy on pallet count (11 vs 12 on manifest) — reported to supplier
Follow-up needed?Yes — awaiting supplier confirmation

How CrewSignals replaces the static template

A downloaded template works until more than one shift needs it at once. CrewSignals keeps the same fields — priority alerts, open issues, follow-ups — as a structured, searchable record, with acknowledgment tracked automatically instead of chased down after the fact.

FAQ

How often should I add an entry?

Whenever something happens that someone might later ask about — deliveries, incidents, visitor access, equipment issues. Routine, uneventful stretches don't need an entry.

Is a shift log the same as a handover?

No — the log is the raw, timestamped record built during the shift. The handover is the distilled summary written for the next shift, usually pulled from the log's flagged entries.

Paper log or digital?

Paper works for a single site with no need to search history. Once you need to find what happened last Tuesday around 2pm across multiple people, a digital, searchable log is much faster.

What's the biggest mistake teams make with shift logs?

Writing entries in a batch after the fact — details get lost or generalized, and exact times become guesses.
