End of Shift Report Template
Use this template to close out a shift with a clear record of what happened, what was completed, and what the next shift needs to know.
Write it at the end of the shift, while it's fresh
Separate "what happened" from "what the next shift needs to do"
Keep metrics to the ones someone will actually read
What to include
- A short summary a manager can read in under a minute
- Completed tasks, so progress is visible without asking
- Incidents and exceptions with what happened and how it was resolved
- The handful of metrics that actually matter for this shift
- Notes for the next shift, kept separate from the historical record
The template

| Incidents / exceptions | 10:42 — conveyor belt #2 jammed, cleared in 8 min, no injury |

| Handoff notes for next shift | Belt #2 still noisy — flagged for maintenance check |
| Prepared by / reviewed by | Prepared: T. Nguyen — Reviewed: supervisor initials + time |
How CrewSignals replaces the static template
A downloaded template works until more than one shift needs it at once. CrewSignals keeps the same fields — priority alerts, open issues, follow-ups — as a structured, searchable record, with acknowledgment tracked automatically instead of chased down after the fact.
FAQ
Is an end-of-shift report the same as a handover?
They overlap but serve different readers. The report is the historical record (for management, audits, or shift comparisons); the handover is action-oriented instructions for the next shift.
How much detail should incidents get?
Enough for someone who wasn't there to understand what happened, what was done about it, and whether it needs follow-up — usually one or two sentences per incident.
What metrics should I track?
Only the ones tied to a decision. If nobody adjusts staffing or process based on a number, don't track it just because it's easy to count.
Can I search past reports for a specific incident?
Not easily in a spreadsheet, once you have more than a few weeks of history — that's the main reason ops teams move end-of-shift reporting into a shared, searchable system.
