Use this shift changeover form when manufacturing or ops language is "changeover" — transfer line status, tools, and open work between named people with dual acknowledgment.

## Form fields

- **Changeover date & time:** 2026-07-26, 18:00
- **From / to (leads):** From: D. Marsh → To: P. Ibe
- **Line / area status:** Line 2 at 80% — bearing swap next PM window
- **Tools / access transferred:** Radio #4, lockout key set B
- **Open work (% complete):** Filter swap Unit C — 60% — parts staged
- **Safety notes:** Wet floor bay 3 — caution tape still up
- **Acknowledgment (both):** From: sign/time — To: sign/time

## How to use it

- State status as of now, not shift start.
- List transferred tools individually.
- Say safety notes out loud in addition to writing them.
